Documents that should accompany the application form:
Certified photocopy of the person’s ID (or Family Book, or Birth Certificate). For foreigners: a certified photocopy of the Residence Card.
Certified photocopies of the medical or psychological reports to prove the disabilities that are being claimed. In case of renewal for aggravation or improvement of the condition: certified photocopy of medical or psychological report to prove the new condition.
When applicable, certified photocopy of the representative or guardian’s ID and of the document that certified the aforementioned representation.
There are other documents that also need to be submitted to the social services, as they will determine economic and work factors:
Certificate of living-together family,
Proof of income of the person and of the people who live with him/her (photocopies of IRPF, payslips, etc.)
Proof of expenses, if applicable (living expenses, other long term expenses- education, health, other- )
