2.4.6 Which documents should you submit?
- Properly filled application form.
- Documents that should accompany the application form:
- Certified photocopy of the person’s ID (or Family Book, or Birth Certificate). For foreigners: a certified photocopy of the Residence Card.
- Certified photocopies of the medical or psychological reports to prove the disabilities that are being claimed. In case of renewal for aggravation or improvement of the condition: certified photocopy of medical or psychological report to prove the new condition.
- When applicable, certified photocopy of the representative or guardian’s ID and of the document that certified the aforementioned representation.
- There are other documents that also need to be submitted to the social services, as they will determine economic and work factors:
- Certificate of living-together family,
- Proof of income of the person and of the people who live with him/her (photocopies of IRPF, payslips, etc.)
- Proof of expenses, if applicable (living expenses, other long term expenses- education, health, other- )
- For unemployed applicants (job seeker card).
